This is jason again,
I realized that I failed to mention that I had the same symptoms as your son and it continued from age 2-6 and worsened as time went by. I was circumcised at birth and the
circumcision may have been flawed causing internal damage that led to priapism. Or an injury to my penis may have occured in some simple accident. My parents were not gentle to my erections , they often forced them down or bent them into my pants to dress
me rather than letting my penis soften first. I kow it would be embarrasing to take your son to a medical facility because his penis is hurting and I am thankful that your trying to help him. He is blest to have a caring father. My father was insistant that my penis be removed to "cure" my priapism and ,illegally, he got his wish. So I am a living survivor of
a penis amputation since 6.5 yrsold.

I not a medically trained source but this is an area of my personal study because I was was a child of 2yrs old when I started suffering with a lot of pain in my penis this aparently
was priapism. If you have not studied priapism, I beg you too because with the knowledge of it you can prevent penis damage from lack action or trying the wrong treatment. I recently found a medical source that stated ages 5 to 10yrold boys are in an age bracket that has a greater frequency of priapism. I remember that my penis hurt during erections and my erections lasted at least 10 hours on several occasions. The pain would continue after the erection ceased but if the erection didn't return for about a day the pain would gradually diminish. At the next moment the next erection started I felt the pain build up again. Now a very important warning - A child's penis is easily injured while an erection is forced to bent out of the natural position of the erection. NEVER BEND A CHILD'S ERECTION! The safest action is to let the erection be free to go to its
erection position, the child should avoid risky play (such as letting objects impact his penis) Very loose clothing is acceptable or cover erection with a sheet only. If no impact or bending occurs during the erection the penis will not be injured unless the erection continues  for too long of time. 4hrs is too long for an adult's penis to be erect, about 1.5hrs of time is long enough to cause injury to a 5yrold's. In either case (long time or bending/impact) the penis is injured and the injury will cause pain. Medical treatment for the injury is crucial or priapism may worsen or continue till penis suffers serious damage.
The serious damage is painful until nerve damage has left the penis numb. The erections that should normally occur may cease or diminish considerably. Many times serious damage causes erectile disfunction immediately or in adulthood.
Hope your son gets well.

Hi there,
Thank you for your question.
Painful erections are very uncommon caused due to recent penile trauma. The condition of an erection which last for more than four hours is called priapism. The condition develops when blood in the penis becomes trapped and unable to drain. If the condition is not treated immediately, it can lead to scarring and permanent erectile dysfunction. Long lasting erections may associated with pain due to the lack of oxygen to the penis because the penis is filled with oxygen deplete venous blood. Peyronie’s disease (a scarring of some of the deep tissues in the penis) can have a localized area of pain but usually involves a lump and some deviation of the erection to the affected side. Erectile pain secondary to psychological disorders is also a possibility. If symptoms continue seek urological help.
This information is provided for general medical education purposes only. Please consult your physician for diagnostic and treatment options pertaining to your specific medical condition. Please remember that priapism is a general emergency so please seek medical help if this problem continues.
Sincerely
Dr Sylvester, MD
