Deep cut on perineum that won't heal!!

About a month ago whilst having sex with my partner, my perineum tore. This has happened before but not to this extent, as this cut is quite deep. Normally they simply heal within a few days - a week, but this time it has been over a month and there are no signs of healing! I am currently volunteering in a developing country, with no access to a doctor, but it stings when I go to the toilet, and the pain when I sit down is unbearable!! I was wondering if anyone had any ideas on how to help the healing process go a bit faster, without having to visit a doctor?

It's very important to keep the area clean- if you have access to it, witch hazel can be helpful to place there too.  Other wise, a doctor would be the best option, but if you don't have access to one, that makes it a lot harder.  Are you seeing any signs of infection, or is it just not healing?
