Pain for 4 months - Behind Testicle and base of Penis

I'm 36 years old.  For the last 4 months, I'm having continuous dull pain behind my top left testicle (behind the base of my penis).   The pain worsens when I sit in cushion seats (where the cushion touches my scrotum).  I'm having mild tenderness while touching, sometimes it becomes more painful.   I'm having pain in the base of my penis as well.    The pain radiates to my entire left left, sometimes, I get pain on the right side leg as well.   I have lot of spasm all over body due to this pain in my genitals and anxiety.

I never had any wounds, sores, lesions or any outbreaks in my genitals anytime.   I met several urologists.   All my STD reports, MRI, Scan, Blood count everything is negative and normal.

When a urologist did a rectal examination to see if I have Prostatitis, I got severe pain in my abdomen, and he says I have Chronic Prostatitis.

Though I never had any sexual contact for several months, and never had any outbreaks, lesions or sores in my genitals, both IgG and IgM for HSV-2 was negative, the observed value for IgG is negligible with a value of 0.1 and IgM was showing an observed value 0.9 which is still negative after several months of my last exposure.   I read several labs have stopped IgM tests for adults as it shows lot of false positives.  Since the IgM was showing 0.9,  though it is negative, for the benefit of doubt, the doctor prescribed me some antivirals for Herpes.   It did not have any effect on me.   The pain continued even after taking medicine for Herpes.     I took IgG tests for 3 consecutive months in different labs, it always came negative with a negligible value of 0.1.    This test has created  lot of stress and anxiety for me.

None of the medications worked for me.
Cipro, Levofloxacin - No effect
Naproxen and other pain killers - No effect
Antivirals - No effect.

Ultimately no medicine is working for me.    Both my life and work is in shambles now.   Wasting too much money for treatments, but unable to find a proper cure for this.     Can anyone help me the ways to recover from this chronic pain which I'm having for the past 4 months?

I hope you found help by now. I was going to ask of anyone ever suggested you could have pudendal neuralgia or Pudendal Nerve Entrapment? You will need a referral to a neurologist who can test for this. If diagnosed there are treatments and surgeries.

As I said my life and work is in shambles, this pain has de-motivated a lot, and sometimes I feel I'm helpless.   I stopped working out for the past 4 months as well as stopped playing tennis due the fear.

Can someone help exactly understand what I'm going through now and what are the treatments available?

I was told by the Herpes forum, the symptoms I have continuously for months with IgG always showing 0.1 is certainly not Herpes.   I was asked to look for the symptoms of CPPS, and try physiotherapy and exercises for my pelvis and prostate.

I have the following symptoms:

1. Sitting triggers or exacerbates discomfort/pain/symptoms
2. Pain or discomfort during or after ejaculation
3. Discomfort/aching/pain in the rectum (feels like a “golf ball” in the rectum)
4. Discomfort/pain in the base of the penis
5. Ache/pain/sensitivity of testicles
6. Suprapubic pain (pain above the pubic bone)
7. Perineal pain (pain between the scrotum and anus)
8. Coccygeal pain (pain in and around the tailbone)
9. Low back pain (on one side or both)
10. Groin pain (on one side or both)
11. Reduced libido (reduced interest in sex)
12. Anxiety about having sex
13. Anxiety and catastrophic thinking
14. Depression
15. Social withdrawal and impairment of intimate relations
16. Impairment of self-esteem
17. Muscle spasm all over the body - due to anxiety
18. Pain in legs

Though I saw the following symptoms are common for CPPS, I DON'T have these.

1. Urinary frequency (need to urinate often, usually more than once every two hours)
2. Urinary urgency (hard to hold urination once urge occurs)
3. Dysuria (pain or burning during urination)
4. Nocturia (frequent urination at night)
5. Reduced urinary stream
6. Sense of incomplete urinating
7. Hesitancy before or during urination
8. Discomfort or relief after a bowel movement

I pissed in my bed on 2 occasions in the last 4 months, which is really embarrassing, I never had the history of pissing anytime from my childhood in this 36 years.   When I checked with the doctor, he was saying it was more due to anxiety.    However, I never had urinary frequency anytime in these 4 months, mostly I won't get up in the nights for urination.
