Anyone had success healing from/treating penis damage to the PSL or nerves?

Hi all.  I've been reading a lot of the posts about penis injuries and nerve damage and decided to share my story and see if anyone has any helpful advice.

About a year ago I was having sex with my girlfriend and she moved into a position that put a lot of strain on the base of my penis. It was kind of painful, and only lasted for about a minute or two, but I didn't stop it. Since then, I have slowly noticed changes to my sexual function:

I've noticed that my penis is now a lot more "floppy" when flaccid. It doesn't have the support it used to at the base and it hangs straight down. I've done some research and it seems like what happened was that I stretched or completely tore my penile suspensory ligament (PSL). Other symptoms to support my self-diagnosis of a torn PSL is that when I'm erect my penis hangs down rather than standing straight up like it did before.

I also have some pain associated. Sometimes after ejaculation my penis feels "sore." This can last for a few hours but can also last multiple days. It's kind of like an achy pain. I also sometimes have pain while erect. The pain I experience is not always consistent or the same. It varies in frequency, duration, and location in the penis. And sometimes I have no pain at all.

I've also experienced an impact to the quality and frequency of my erections. They vary in their hardness, but are pretty consistently softer than they were before. I also have to pretty much maintain consistent contact/stimulation to keep them. I did go see a urologist who gave me a prescription for Cialis. I took one once and I was able to have a full rock-hard erection, but it still felt unsupported at the base. I also do not really have spontaneous erections anymore throughout the day. The last time I had one was a few months ago.

Another thing I've experienced is some decreased sensitivity in the penis while it is erect. I don't always get morning erections anymore, but when I do I've noticed they're not as hard and I don't feel them as erections. I know that's hard to explain, but they don't have as much sensation. I'm afraid the injury affected the nerves running to my penis.

I feel really alone and helpless about it. It seems doctors don't really have a ton of answers and are quick to just want to treat the erectile dysfunction with prescription drugs. The doctor I first saw told me that the injury and the ED were separate, but I think they're related. But I want to actually fix the problem, or at least know what happened. I'm hoping people can share some success stories or some similar experiences they've had and things they've done to regain their sexual health.

If there are any doctors out here or medical professionals, please reach out. I'll check this often and try to keep the conversation going. Relevant information that might be helpful for understanding the situation. I have a slight natural curve to my penis that is straight up. It has not been a problem or factor before the instance I mentioned above.

I'm also going to update my progress and my medical visits so that you all know what the doctors are saying and what tests are being run to rule out certain things. I've seen a lot of really good people on these forums try to help, and I want to provide as much help as I can to anyone else suffering with uncertainty around these kind of issues.

have you gain any improvement from the penile nerve issue and the weak erection as before? hope that seeing different urologists will allow you to recover better.
my glans has no pleasure feeling when rub or masturbation.

Hi Aaron, yes I have made some really great progress in the last 8 months. First, because I was scared I spent a lot of time talking to urologists and andrologists. I went to the Mayo Clinic and had tests done to make sure I didn’t have any structural damage or problems. I talked to Dr. Ralph in London over the phone (cost me about $300). I also talked to a sexual health/medicine specialist in San Diego on the phone. After confirming there wasn’t something wrong with the actual function of my penis, one of my doctors actually told me to go to a physical therapist who specializes in the pelvic region. I found an awesome one who also specializes in male pelvic floor physical therapy. There I was diagnosed with pelvic floor dysfunction and pudendal neuralgia. Pelvic floor dysfunction means that the muscles in my pelvic floor - the ones that support sexual function - were tight/shortened, and had been putting pressure on my pudendal nerve. Most of my pain, loss of sensitivity, and quality of erection, was the result of compression of that nerve. Once I started to go to physical therapy, she was able to start to release those deep internal muscles and remove the compression on the nerves. For the first 2 months I went every week. Since then I go every 10-14 days.

It sounds good for you to make an advancement towards the goal of recovery. The pelvic nerve obstruction is very much aware after I came across a book about how pelvic muscle exercise can heal the erectile dysfunction during a visit to a library. It was a about six months ago. All I did was follow each type of exercise described by the author. I recalled getting a slight pleasure feeling from morning wood and without being touched, the previous night doing this exercise.
I have to seek out the real therapist or specialist and let the expert do their jobs. Like what you did, seeing the actual person upfront.
From now onwards, I am going to explore this therapy more. Thank you.

I went to see the urologist last week. listening to him talking about possibility of these and those that could be the cause of the loss of sensation. eg. desensitization, change of diet, aging, spine injury, bent penis, depression, stress, psychological issue, etc, all can be googled on the internet. In the end, no solution was given. I walked out empty handed. Also he told me it's not pelvic muscle issue, nothing related to that, immediately after I mentioned about it. I suggested to him other possible solution such as acupuncture, pelvic muscle therapy and Chinese traditional herbal medicine. Well, of course he wouldn't recommended these alternative because they are all totally outside his line of work.

As part of a self initiated move, I took the leap of faith by moving out my old place and staying in a different city by myself. Hoping this action would change somehow mentally/psychologically how my penis system work, and return to normal.
