In the hands of a skilled tech it is safe. There are two methods of permanent makeup, one way is done with a rotary tool and the other is with a manual tool. I have had it done both ways and prefer the manual myself. The difference between the methods are: rotary method can be very painful and fading more because the machine goes below the top two layers of skin where the body sees it as foreign matter and breaks down the ink. The manual way isn't painful at all and sometimes you may never need a touchup, this way only applies the ink in the top two layers.
Make sure that the salon that you go to offers a consultation a week before your procedure for an ink test to make sure your body will accept the ink properly.
I have had my eyeliner for 3 years using the mannal method and no fading whatsoever and I absolutely love it!
