seronegative RA

How can one determine if a patient has RA if there is no RF and no synovitis in any of their joints?  No significant morning stiffness, no elevated SED rate or CRP?  Most DMARDS other than NSAIDs have failed to make a difference, but a rheumatologist says they have RA.
